For Discharge Planners and Case Managers

You know the discharge that worries you. The patient is medically ready to leave, but they need insulin managed, or a wound dressed, or medication given correctly, and there is nobody at home who can safely do it.

Veritas provides nurse-delegated care across King County, which means a registered nurse assesses the patient, writes the plan, trains the caregiver on those specific tasks, and continues to supervise. It is designed for exactly that gap.

What We Can Take On

Under RN supervision, our caregivers can support:

  • Medication administration, not just reminders
  • Insulin injections and blood sugar monitoring
  • Wound care and dressing changes
  • Ostomy and catheter care
  • Tube feeding
  • Bowel programs
  • Personal care, meals, mobility support and companionship

What We Cannot Take On

Being straightforward about this saves everyone time.

Delegation requires the patient's condition to be stable and predictable. If needs are changing rapidly or require continuous clinical intervention, skilled nursing or a facility remains the right call, and we will tell you that directly rather than accept a referral we should not take.

How Fast We Can Start

Most referrals begin with a nurse assessment within 24 to 48 hours, and care typically starts within days. If a discharge is time-critical, call us and we will tell you honestly whether we can meet it.

How to Refer

Call 425-541-8281 and ask for the care team. Tell us the situation, the discharge date and the clinical needs. We will confirm quickly whether we can take it, arrange the nurse assessment, and keep you updated on whether care started.

We will not leave you wondering whether a referral landed.

When a Community Is the Better Option

We also provide senior placement guidance, at no cost to families. If a patient would genuinely be better served in assisted living or memory care, we will say so and help the family navigate it.

That honesty is why referral partners keep working with us.
